ABBR with title Screen reader compatibility

Expected Result: works in a screen reader

Actual Result: causes problems in 36 screen reader / browser combinations

Note: some screen readers have speech dictionaries mapping common abbreviations to their spoken equivalents. For example, "etc" is usually mapped to "et cetera" when spoken in NVDA and JAWS. This can give the impression the `abbr title` attribute is being spoken when in fact the speech dictionary is being used. JAWS has an 'Expand Abbreviations' setting which does read `abbr title`, but this is turned off by default.
